TIPS FOR PASSING YOUR MEDICAL MARIJUANA EVALUATION

Regardless of which state you live in, if you want to apply for a medical marijuana card, you are going need to go through your state’s application process. While some states have an easier process than others, most states have the same requirements.

You will need the following information to successfully obtain your Oklahoma medical marijuana patient card.

  • Proof of Oklahoma residency (driver’s license or state-issued ID card are preferred)

  • Proof of identity (driver’s license or state-issued ID card are preferred)

  • A working email address

  • A clear, color, full-face digital photograph. Please do not zoom in and make sure your shoulder's are showing. NO jewelry or smiling please. More information below:

  • If applicable, the patient’s Medicaid (Sooner Care), or a Medicare insurance card or enrollment documentation. OMMA will need proof of your enrollment.


1. Government ID (proving that you are a resident of the state). If you don’t have a state ID, then make sure you bring with a utility bill proving residency.


2. Upload a picture on a white background. Do NOT smile. Use a digital camera that will provide a clear, digital photography. Do not zoom in and shoulders must show as stated above. 
3. Photos must be taken vertically, not sideways. 
4. File formats to upload are .jpg., .png or .gif and no larger than 3 MB in size.
